Welcome to the Well Street Care Management website. As you can see, we’re still under construction, so we apologize for the dust. The Well Street website will serve as a place to share valuable information, to read about new clinical programs and insights, and to discover the latest news.
Well Street Care Management designs, delivers and administers innovative, cost-effective, patient-centered medication therapy management (MTM) services through its statewide network of pharmacists and pharmacies. The vision for Well Street Care Management is to improve the care patients receive through evidence-based clinical practice; to reduce overall healthcare costs; and, most importantly, to promote the connection between patients and their pharmacists. Well Street was founded by the Michigan Pharmacists Association and its for-profit subsidiary Pharmacy Services Inc.
